No matter what the primary cause of your knee pain when squatting is, movement and targeted exercises are usually a first-line treatment for each of these issues. They help strengthen the knee to reduce pain and prevent pain flares from occurring in the future.

  • Patellar tendinitis. The patellar tendon is located just below the kneecap. Repetitive force through the patellar tendon from activities like repeated jumping (often called jumper’s knee ) can contribute to inflammation or swelling just below the knee. If the quadriceps tendon becomes strained or irritated, you’ll feel pain just above the kneecap.
  • Patellofemoral syndrome. This condition can cause a dull ache behind or around the kneecap and is caused by irritation in those areas. It may get worse during activities such as squatting and climbing stairs. It can also be referred to as runner’s knee because it’s common among athletes, but anyone can develop patellofemoral syndrome, regardless of activity level.
  • Meniscus tear. Just as it’s normal to develop gray hair on your head or wrinkles on your skin, it’s normal for the cartilage that cushions the knee joint to change with age. While these changes are often unnoticeable, they can contribute to knee pain, stiffness, swelling, or reduced range of motion for some people. You may also feel a catching or locking sensation.
  • Osteoarthritis. Similar to the meniscus, it’s normal for the articular cartilage that cushions the knee joint to change with age. If these changes contribute to friction in the knee joint, you may experience aches or stiffness that can make everyday movements, including squats, more difficult.
  • Weak glute muscles. Research shows that working the glutes, inner hip, and inner thigh muscles can help reduce knee pain. If you have pain with squatting and your knees cave inward during a squat, try strengthening your glute muscles to see if it helps, suggests Dr. Anderson. Start with the standing side leg raise (instructions below). Know that knees caving inward during a squat isn’t necessarily a bad thing, or something that contributes to pain, but it may indicate a lack of glute strength which can be a factor in the bigger picture of your knee pain.
  • Limited ankle mobility. Everything in the body is connected. So if your ankle has a limited ability to flex forward or adjust side to side, it ultimately affects the angle at which your knee bends and it may force your knee to assume a position that’s more stressful for you. Simple at-home exercises can improve your ankle’s range of motion (see prevention tips and exercises below).
  • Too little movement throughout the day. Staying in the same position for too long can be irritating to your joints and make them stiffer. There’s no right or wrong way to sit or stand throughout the day. The most important thing you can do to prevent and manage stiff joints is to change positions and move throughout the day. Does deep squat cause knee pain?

Abstract – It has been suggested that deep squats could cause an increased injury risk of the lumbar spine and the knee joints. Avoiding deep flexion has been recommended to minimize the magnitude of knee-joint forces. Unfortunately this suggestion has not taken the influence of the wrapping effect, functional adaptations and soft tissue contact between the back of thigh and calf into account.

The aim of this literature review is to assess whether squats with less knee flexion (half/quarter squats) are safer on the musculoskeletal system than deep squats. A search of relevant scientific publications was conducted between March 2011 and January 2013 using PubMed. Over 164 articles were included in the review.

There are no realistic estimations of knee-joint forces for knee-flexion angles beyond 50° in the deep squat. Based on biomechanical calculations and measurements of cadaver knee joints, the highest retropatellar compressive forces and stresses can be seen at 90°.

  1. With increasing flexion, the wrapping effect contributes to an enhanced load distribution and enhanced force transfer with lower retropatellar compressive forces.
  2. Additionally, with further flexion of the knee joint a cranial displacement of facet contact areas with continuous enlargement of the retropatellar articulating surface occurs.

Both lead to lower retropatellar compressive stresses. Menisci and cartilage, ligaments and bones are susceptible to anabolic metabolic processes and functional structural adaptations in response to increased activity and mechanical influences. Concerns about degenerative changes of the tendofemoral complex and the apparent higher risk for chondromalacia, osteoarthritis, and osteochondritis in deep squats are unfounded.

With the same load configuration as in the deep squat, half and quarter squat training with comparatively supra-maximal loads will favour degenerative changes in the knee joints and spinal joints in the long term. Provided that technique is learned accurately under expert supervision and with progressive training loads, the deep squat presents an effective training exercise for protection against injuries and strengthening of the lower extremity.

Contrary to commonly voiced concern, deep squats do not contribute increased risk of injury to passive tissues.

So if you truly want to strengthen your lower body and legs, then you want to be strong throughout the full range that your body can squat.

If you are only performing partial squats, such as to 60 degrees, then you will only be strong within that partial range. When you are at a lower depth, more muscles are activated and recruited, so you are strengthening even more muscles of the lower body and legs. Squatting deeper will help increase athletic performance such as jumping, and with day-to-day activities, such as getting up from the floor and lower chairs.

‍Now for the more complicated answer. There are instances when someone cannot squat deep. People with injuries and post-surgery will of course have to be careful with how low they squat. Someone just starting to perform squats will not have the motor control, mobility, and strength to go fully down.

Some people naturally will not have the mobility to perform a full depth squat without compensating with some part of the body. While mobility is something that can be worked and improved on, there can still be limits for some. What this means is that there is no straightforward answer to how deep you should squat, but rather how low you go depends on what your body is able to do without putting yourself at injury risk to your back or other body parts.

While the goal is to be able to go as low as possible, you only want to do so within a safe limit. It is important you assess and listen to your body to find out what your body is capable of. Every person’s body is unique and different, and thus, our approach to how we treat and exercise should always be individualized and tailored to what fits each unique individual.

Your knees may not ache as much if you put less weight on them when you squat down to reach for files at work or pans in the kitchen.

Some people might even say a workout was only good if it leaves you sore. What we are talking about here is what scientists call Delayed Onset Muscle Soreness (DOMS), and it’s something anyone who exercises regularly has probably experienced at some point. What is DOMS? No one knows the exact source of DOMS, but experts believe it occurs because damage takes place in the muscle or nerve that was stressed during the workout.

It seems to happen in three situations: 1. A person does a new, unfamiliar exercise.2. A person goes back to an old workout after having not done it for a long time.3. An exercise puts load on a muscle that is lengthening, which is otherwise known as an eccentric muscle contraction.

  • Regardless of what’s happening at the cellular level, there are some things to consider about what DOMS does to your body, and to your performance. People with DOMS often experience the following symptoms:

Because DOMS is an uncomfortable experience, it can lead to reduced performance. Because of this, when you are competing, it is generally recommended that you reduce the intensity of your workouts. So how much soreness is too much? This is a difficult question to give just one answer to, because the answer depends on you: your training experience, your pain tolerance, your sport, and everything else you intend to do throughout the day after working out. Some people have questioned whether DOMS is a sign that a workout was effective, but evidence does not support this premise. DOMS is a sign that some level, damage has occurred, and some damage is necessary for muscles and nerves to adapt. But people have very different reactions to workouts, and some muscles are much more prone to feel sore than others.

DOMS just isn’t consistent enough to be a good measure of workout quality. Soreness is considered normal if it occurs between 24-72 hours after a workout, and if it does not prevent you from completing normal daily activities. If it lasts longer than this, or is so intense that it prevents you from functioning normally, it could be a sign of significant damage.

I’m Sore: What should I do? Because DOMS happens on a regular basis during training, people have considered many ways to reduce its intensity. Non-Steroidal Anti-inflammatory drugs (NSAIDs) like Ibuprofen have been demonstrated to reduce DOMS. Massage has also been demonstrated to reduce DOMS if it is performed right after exercise.

  • Cryotheray (cold therapy, or using a cold pack), stretching, and electrical current modalities have demonstrated no effect on DOMS.
  • Exercise is considered the most effective way to reduce DOMS, but the effect is temporary.
  • The most common method people use to reduce DOMS is to simply wait; the discomfort and lost performance will reduce as your body recovers.

Any good workout regimen should challenge body and allow adequate recovery. Too much challenge, your body can never adapt to the activity. Too little, the workout wasn’t hard enough to make you adapt. Too little rest and your body does not have time to recover.

It’s the umbrella term for pain felt behind your kneecap, where your patella (kneecap) articulates with your thigh bone (femur). This joint is known as your patellofemoral joint. Patellofemoral pain syndrome is the result of excessive pressure between your knee cap and thigh bone from poor patellar alignment, which over time affects the joint surface behind the kneecap and leads to dysfunction and difficulty participating in the things you love.

In a healthy, pain-free knee, the patella glides up and down through a groove in your thigh bone as you bend and straighten your knee. As the knee bends, the pressure between your kneecap and the groove increases. This is why activities involving deep knee bends like squats and lunges are two of the more common complaints by people suffering from patellofemoral pain.

The patellofemoral joint pressure is further increased if the patella ‘doesn’t track in an ideal fashion’, meaning it glides in a non-ideal alignment and rubs against the femur (thigh bone). When this occurs pain is often more severe and debilitating and may be felt even when resting or sitting. The most common causes of patellar mal-alignment are an increased spike in loading including hills or speed work, abnormal muscle imbalance and poor biomechanical control.

W e see this on assessment when the knee wants to collapse inwards during activities such as a single leg squat, with wasting of the inner quadriceps muscle on observation, or poor pelvic alignment and control on tasks including squatting, lunging and jumping.

  1. Manage your load. If you’re battling knee pain unfortunately you’ll need to swallow your pride and incorporate rest days and ensure hill and speed work are kept to a minimum whilst things settle.
  2. Strength train: research shows exercise of the hip and thigh twice a week resulted in better outcomes than hands on therapy including massage and dry needling. We preach it cause it works – get strong to feel better. Strength work can be used in conjunction with taping and other modalities even in the acute stage to help settle symptoms as well as address the underlying cause for your pain.
  3. Seek to improve hip and knee biomechanics when running. This will normally improve somewhat with strength training and improving strength in the right areas, however some small alterations to running technique through verbal cuing may be required also. We take the approach “don’t change something that’s not broken”. There are things that are ‘better bang for buck’ than altering running technique such as strength training and load management, but if you’re a runner and things are not progressing as we’d like, small tweaks to your running technique may be necessary.
  4. Be careful of “rolling your ITB”. The ITB is a passive structure and you can roll it until the cows come home, it’s not going to change length. Focus on your outer quad, hip and glute instead for short term relief.
  5. If you’re a runner, focus on quicker, shorter steps when running. In running terms, this is called increasing your cadence, Research shows a 5-10% increase in your cadence can reduce the force on your knee by up to 20% and can lessen your discomfort as a result.
