Orthostatic headache is a medical condition in which a person develops a headache while vertical and the headache is relieved when horizontal. Previously it was often misdiagnosed as different primary headache disorders such as migraine or tension headaches. Increasing awareness of the symptom and its causes has prevented delayed or missed diagnosis.

A secondary headache disorder is more often suspected than detected. Many headache patients undergo neuroimaging without detecting anything of clinical relevance. Only 11 had an abnormality (2.1% of patients scanned and 0.3% of the total study population).22 A retrospective study reviewed 360 patients referred for MRI evaluation of a chronic or a recurrent headache.24 Only 0.7% of the patients had relevant findings on MRI.24 In a review of 328 patients with a nonfocal headache, only 1.5% had clinically relevant results.2 A review of 402 chronic headache cases identified abnormalities in 3.7%.25 These studies reveal that even with the use of red flags, most scans still return no findings.

- In 2003, the mnemonic SNOOP (systemic symptoms/signs and disease, neurologic symptoms or signs, onset sudden or onset after the age of 40 years, and change of headache pattern) was proposed as a red flag detection tool for secondary headaches.7 National and regional guidelines 8, – 11 have since provided more items to screen for potential secondary causes leading to the current SNNOOP10 ( table 1 ).

The scalp may become tender and extremely sensitive to the point where a light touch can cause severe pain (allodynia).
