What causes testicular pain and swelling? – Many testicular problems cause both pain and swelling. Sometimes, the pain can be dull or achy and worsen over time. Testicular pain can also be sudden and severe and can make a child’s stomach upset. If your child has sudden, severe pain in the testicles, it’s important to see a medical provider right away.
Testicular torsion – A serious condition that happens when the spermatic cord twists and cuts off blood supply to the testicle. Torsion of the appendix testis – A problem that occurs when a small piece of tissue on top of the testes twists and causes swelling and pain.
Other conditions that cause testicular pain and swelling include:
Infection (epididymitis) – A condition that can cause swelling in the epididymis, a tube behind the testicles that helps activate sperm. Hernia – You may notice a hernia as a bulge above the testicle. The bulge can sometimes extend into the scrotum and cause mild, moderate or severe pain. Varicocele – A condition that occurs when there is swelling in the vein that drains blood from the testicles. It can cause a dull ache and swelling and may feel like a bag of worms above the testicle. Hydrocele – A condition where fluid gathers around the testicle. While it may not cause serious problems, it can result in uncomfortable swelling. Summer scrotum or summer penis – A common summertime condition that causes the skin around the scrotum or penis to become red, swollen, itchy and painful. It may look and feel like an allergic reaction or bad bug bite. Orchitis – An uncommon condition that causes the testicles to swell.
The treatment for testicular pain and swelling depends on the type of condition you have. Treatments can include antibiotics, surgery and careful monitoring.
Can hot weather cause testicular pain?

Heat stress is a harmful factor for many biological systems in the body, such as the circulatory system, integumentary system, respiratory system, and male reproductive activity (2).

It has been determined that spermatogenic cells are sensitive to high temperatures (2). After testicular heat stress, many cells undergo extrinsic and intrinsic apoptosis signal pathways (5). Spermatogenic cells are affected by heat stress principally by primary spermatocytes and early spermatids (6).
- In a previous study, the heat-exposed mice showed degenerative changes with spermatic arrest in most of the seminiferous tubules (7).
- Heat stress has also been shown to increase sperm DNA fragmentation (8).
- The mechanism of sperm DNA damage may involve various types of oxidative reactions, DNA repair errors in the late stages of spermatogenesis, and functional abnormalities that reduce the protective ability of Sertoli cells (9).
Heat stress adversely affects human and animal health, including through an increase in the inflammatory process in the body, largely in mammals (10). Several studies have shown that heat stress increases inflammatory responses and the release of inflammatory mediators such as cytokines in the kidney, muscle, and intestine (11, 12).
- Heat stress has been shown to systemically increase inflammation signals through the pathways of toll-like receptor 4, activator protein 1, and nuclear factor kappa-light-chain-enhancer of activated B (13, 14).
- Several studies have shown that heat stress affects the induction of fibrosis in various tissues including muscle, liver, and lung (15-17).
Heat stress increases the excessive accumulation of extracellular components, in which collagen plays an important role, and this process often occurs after trauma; inflammation leads to increased fibrosis, resulting in reduced function and causing chronic diseases in many tissues and organs (18).

- Rest: Limit your activity until your pain decreases. Get more rest while you heal. Do not sit for long periods of time.
- Cold packs: Place cold packs on your testicles to help ease your pain. Use cold packs as directed.
- Elevation: Gently tuck a folded towel under your testicles to lift them as you sit in a chair or lie in bed. This will help ease your pain and decrease swelling.

Can heat cause testicular torsion?
Abstract – Purpose: The risk factors for acute testicular torsion are poorly understood. Environmental factors have been implicated by some authors and discredited by others. The only previous known study in the United States did not demonstrate any seasonal correlation, although the study was done in a warmer climate. We sought to determine if environmental factors impact the incidence of testicular torsion in our temperate climate conditions with a large differential between the warmest and coldest temperatures. Materials and methods: We retrospectively studied patients who were diagnosed with testicular torsion between January 1997 and December 2006. Data regarding weather conditions were collected, including season, temperature, humidity and atmospheric pressure at the time of onset of symptoms. Spearman’s rank correlation was performed to assess the relationship between atmospheric temperature and frequency of testicular torsion. Multivariate analysis was performed to analyze the effect of covariables. Results: A total of 58 children presented with testicular torsion. Mean temperature at onset of symptoms was 6.9C (range -12C to 23C). Of the patients 81% had symptom onset when the atmospheric temperature was less than 15C. Seasonal incidence of testicular torsion was 36.2%, 31%, 19% and 13.8% for spring, winter, summer and fall, respectively. Spearman’s rank correlation test revealed a significant negative correlation between the incidence of testicular torsion and increasing temperature (r = -0.94, p <0.0001) and decreasing humidity (r = -0.44, p <0.001). After controlling for effects by patient age, atmospheric pressure and humidity by multivariate analysis a significant correlation was observed between testicular torsion and decreasing atmospheric temperature. None of the other atmospheric factors examined correlated with the incidence of testicular torsion. Conclusions: An increased incidence of testicular torsion is seen with decreasing atmospheric temperature and humidity, suggesting a possible etiological role.
