It weighs about 20-25 grams and has two lobes connected by an isthmus in the middle. The thyroid gland produces, stores, and secretes a hormone called thyroxine which plays a major role in metabolism and helps to regulate various body functions. The thyroid gland produces three hormones:
- Triiodothyronine or T3
- Thyroxine or T4
- Calcitonin
Out of these hormones, T3 and T4 are main thyroid hormones whereas calcitonin is involved in calcium metabolism. Iodine is the main building block of thyroid hormones. The thyroid gland takes up iodine from our food to convert it into thyroid hormones.
The cells of thyroid gland combine iodine and amino acid tyrosine to produce T3 and T4. Then these hormones are released in the bloodstream for the regulation of metabolism (the rate at which the body produces energy from nutrients and oxygen). The production ratio of T4 and T3 by the thyroid gland is 80:20.
However, T3 is 4 times more potent than T4. Two glands in the brain hypothalamus and pituitary maintain the level of T3 and T4 hormones in the blood. One of the major endocrine glands situated at the base of the brain that is pituitary gland secretes certain vital hormones such as TSH (Thyroid-stimulating hormone or Thyrotropin) along with other hormones.
When the level of T3 and T4 hormones (thyroid hormones) is below the required amount, the pituitary gland secretes TSH. TSH then stimulates increased production of T3 and T4 hormones by the thyroid gland. As the T3 and T4 level rise in the blood, the pituitary gland senses this and reduces TSH production.
The hypothalamus is that part of the brain which produces TRH (Thyrotropin Releasing Hormone). TRH regulates the production of TSH by the pituitary gland. Underactive thyroid or hypothyroidism can be categorized into two major types. Primary hypothyroidism: Here thyroid gland itself is at fault and unable to produce sufficient thyroid hormones. There are various causes for the dysfunction of the thyroid gland.
- Secondary hypothyroidism: Here the problem lies at the level of the pituitary gland.

Women are more prone to develop underactive thyroid than men. It is 10-20 times more common in women than men. The treatment protocol for an underactive thyroid should be designed in such a way that it corrects the laboratory values and also make the patient feel better. During the initial stages of an underactive thyroid, increased conversion of T4 to T3 occurs and hence T3 levels are maintained but T4 levels may be low to normal. Causes of Hyperthyroidism – Hyperthyroidism can be caused by a number of things:
Toxic nodule – A single nodule or lump in the thyroid can produce more thyroid hormone than the body needs and lead to hyperthyroidism. Toxic multinodular goiter – If the thyroid gland has several nodules, those nodules can sometimes produce too much thyroid hormone causing hyperthyroidism. This is most often found in patients over 50 years old. In many cases, a person may have had a multinodular goiter for several years before it starts to produce excess amounts of thyroid hormone. Graves’ disease – Graves’ disease is an autoimmune disorder in which the body’s immune system attacks the thyroid. Patients with Graves’ disease often have enlargement of the thyroid gland and become hyperthyroid. In some patients, the eyes may be affected. Patients may notice the eyes become more prominent, the eyelids do not close properly, a gritty sensation and general irritation of the eyes, increased tear production, or double vision. Like other autoimmune diseases, this condition may occur in other family members and is much more common in women than in men. Sub-acute thyroiditis – This type of hyperthyroidism can follow a viral infection which causes inflammation of the thyroid gland. This inflammation causes the thyroid to release excess amounts of thyroid hormone into the blood stream which leads to hyperthyroidism. Over time the thyroid usually returns to its normal state. Because the stored thyroid hormone has been released, patients may become hypothyroid (where their thyroid gland produces too little thyroid hormone) for a period of time until the thyroid gland can build up new stores of thyroid hormone. Postpartum thyroiditis – Some women develop mild to moderate hyperthyroidism within several months of giving birth, which usually lasts 1 to 2 months. This is often followed by several months of hypothyroidism. Most women recover and have normal thyroid function. Excessive Iodine ingestion – Some food sources with high concentrations of iodine, such as over the counter supplements, kelp tablets, some expectorants, amiodarone (a medication used to treat certain heart rhythm problems) and x-ray dyes, may occasionally cause hyperthyroidism in some patients. In most cases, the hyperthyroidism usually resolves when the supplement is discontinued. Overmedication with thyroid hormone – Patients who take too much thyroid hormone replacement can also develop hyperthyroidism. Patients should have their thyroid hormone levels evaluated by a physician at least once each year and should NEVER give themselves “extra” doses unless directed by a physician. Changes in thyroid medication should always be guided by thyroid function testing.

In cases like these, homeopathic remedies consist of nothing but water. TSH and triiodothyronine or (T3) peak at 1:00 -2:00 AM and 2:30 – 3:30 AM respectively with the rhythms directly tied to the SCN master body clock. TSH and thyroid hormone levels change as a result of disruptions to the clock.
Shift workers are more likely to have elevated TSH levels and are more susceptible to autoimmune thyroid problems due to circadian rhythm changes. Obesity, cancer, metabolic syndrome/diabetes and autoimmune disorders are linked with disrupted endocrine body clocks.
TSH signals are affected too by the adrenals, the HPA (hypothalamus-pituitary-adrenal) axis and its signaling hormone ACTH. ACTH is similar to TSH. The hypothalamus and pituitary send out signals to produce TSH and ACTH. TSH signals the thyroid gland. ACTH signals the adrenal glands for cortisol release.
ACTH stim test assesses how well adrenal glands are able to mount a response to stress.
